Commonest type of hyperthyroidism, grave"s disease thyroid gland becomes autonomous, enlarged producing lots of t3 and t4 and shuts down pituitary tsh so plasma tsh quite low, which indicates patient has hyperthyroidism probably of autoimmune grave"s type. Rare case with pituitary failure and not enough tsh, and patient can be hypothyroid for that reason but much less common. As tsh in plasma goes up, this happens since t3 and t4 are falling in patients whose thyroid gland failing. For hyperthyroidism, try to sort out different types, patient may be given radioactive iodide to see how much taken up into thyroid gland, hyperthyroidism (grave"s disease)=doubled uptake of iodide. Cold intolerance=patient who clearly got to stage of hypothyroidism, in summer wearing a hoodie since cold intolerance but this is subjective and not very diagnostic. Commonest cause is type of autoimmune destruction, hashimoto"s thyroiditis.
