As tsh in plasma goes up, this happens since t3 and t4 are falling in patients whose thyroid gland failing. For hyperthyroidism, try to sort out different types, patient may be given radioactive iodide to see how much taken up into thyroid gland, hyperthyroidism (grave"s disease)=doubled uptake of iodide. Cold intolerance=patient who clearly got to stage of hypothyroidism, in summer wearing a hoodie since cold intolerance but this is subjective and not very diagnostic. Commonest cause is type of autoimmune destruction, hashimoto"s thyroiditis.
