Copd case: etiology of copd, cigarette smoking is a primary risk factor, genetic: deficiency in a1-antitrypsin (autosomal recessive, our patient joshua: Irritants: dust, chemicals, pollution: chef (may have been irritating), smokes (primary risk factor), pathophysiology of copd. This is for patients with chronic respiratory failure/end- stage: ensure good nutrition, energy saving techniques when coughing, pharmacological management of copd. Use of bronchodilators (main therapy for copd: initial therapy: saba/sama combination to relieve dyspnea. Salbutamol (100 mcg/puff), 1-2 puffs inhaled tid-qid prn. Equal efficacy in both, side effects in both include tremor, nervousness, hypokalemia, tachycardia, palpitations. Duration of action in both is 4-6 hours. or. Ipratropium: 2-4 puffs inhaled q6-8h. slower onset of action than salbutamol but longer duration of action (8 hrs). The combination of ipratropium (sama)/salbutamol (saba) = most improvement in fev1 & greater bronchodilation & greater improvement in lung function & lower side effects compared with either drug alone.
