May contain black, green, yellow fungal debris: commonly bacterial infections, pseudomonas aeruginosa (20-60%, staphylococcus aureus (10-70%) Symptoms of acute diffuse otitis externa (aoe: acute onset (within 48 hours, produces ear pain/discomfort (otalgia, otorrhea (discharge, pruritus/itching, tenderness, cellulitis of pinna, unilateral in 90% cases, ear canal pain when chewing. Treatment of acute otitis externa: cleaning of ear canal, drops used 3-4x daily, used drops 3 days after symptoms gone, symptoms better in 3 days, gone by 10 days, keep ears dry, take bath instead of shower. Avoid- swimming, water sports until treatment finished: don"t point into ears. Goals of therapy (parameter, desired value, timeframe: eliminate pathogenic microorganisms within 10 days, control pain, fever & otorrhea, restore canal to normal health to resist infections normal acidic ph & adequate cerumen. Chronic otitis externa: most cases due to allergies or inflammatory dermatologic conditions treat underlying conditions, chronic suppurative otitis media when chronic or intermittent otorrhea over weeks-months, w/open tympanic.
