Why are there 2 abx being used: prevent resistance, better efficacy, to cover for other sites (ie pharynx) Repeat screening 6 months post-treatmetn f/u cultures should be done 3-7d after completion of therapy esp if: pharyngeal infection, persistent sx, case treated w/ abx other than ceftriaxone, quinolones were given etc. Least common of the 3 reportable bacteria stis. Outbreaks are seen w/ msms (or sex trade) Note: strong association b/w syphilis & hiv inc. risk of hiv infection (test all pt w/ syphilis for hiv) Spirochetes are helical/spiral m/o that are highly motile come in contact w/ infectious, moist lesions most commonly during oral, vaginal, or anal sex. Spirochetes pass through intact mm & abraded skin after which they access the blood stream. 10-90d after the treponeme develops at the portal of entry a primary lesion forms: painless which persists for 1-5d then heals spontaneously.
