How injury to the nephron leads to attempted repair that may not be successful. Injury causes the kidney to attempt to heal itself, but this is not always done correctly. Misdirected filtration: loss of podocytes (denuded area) causes adherence to bowman"s capsule. Filtrate poured into the interstitium damage (inflammation) De-differentiation: cells are only differentiated (from mesenchymal cells) when in contact with their basement membrane. Damage to the basement membrane cells de- differentiate (become mesenchymal cells again) Each kind of cell in the kidney is capable of overdoing its response scarring how the kidney adapts to structural damage to achieve a new functional steady state. First couple of nephrons are damaged, no change in gfr. Eventually as more are lost (further injury), kidney has a hard time excreting phosphate and reabsorbing calcium - kidney has to work harder to do the same amount of work because of the lost nephrons. Compensates using pth and fgf23 ca reabsorption, phos reabsorption.
