Damage to the basement membrane cells de- differentiate (become mesenchymal cells again) Each kind of cell in the kidney is capable of overdoing its response scarring how the kidney adapts to structural damage to achieve a new functional steady state. First couple of nephrons are damaged, no change in gfr. Eventually as more are lost (further injury), kidney has a hard time excreting phosphate and reabsorbing calcium - kidney has to work harder to do the same amount of work because of the lost nephrons.
