Cue: the subjective and objective data you identify that something is wrong. Data: information; (database: all pertinent patient info collect by the nurse and others. Emergency assessment: when a physiologic or psychological crisis presents itself, the nurse performs this. Focused assessment: nurse gathers data about a specific problem that has already been identified, done during the initial asses if patient"s problems resurface. Objective data: observable, measurable, data that can be seen, heard, felt or measured by someone other than the person experiencing them. Subjective data: info perceived only by the affected person, this data cannot be perceived or verified by another person. In class notes: collect data, subjective & objective, validate: is this true/not, cluster data & look for cues, this tells you what diagnosis you need to come up with, document. Assessing: the systemic and continuous collection, analysis, validation, and communication of patient data or info.
