Humans are the only natural reservoir for this intracellular pathogen. Tb is the leading cause of death in the world from bacterial infectious disease. Roughly 1/3 of the world population is infected (cdc) 1. 3 million deaths in 2012 (cdc) and 9 million cases worldwide in. In us, tb is on the decline since 1992. Also going down in people co-infected w/ hiv. Most new cases (66%) in us are from foreign born people. Primary infection: micro droplets enter the lung. Vigorous cell mediated immune response causes much of the resulting damage: tubercles form (collection of cells surrounded by activated macrophages) At this stage the individual has tb infection, not tuberculosis.
