A focus on choice and actuarial fairness acts to increase total costs while decreasing coverage and patient outcomes for those with health problems. Knowing whether persons in the top percentiles of the health expenditure distribution exhibit high expenditure is fundamental to developing health plan employment policies, containing costs, and understanding the consequences of costly illnesses. Canada has a good healthcare system even though it s not perfect. Reforms to strengthen one aspect are likely to weaken others.
