Oxygen saturation: check for nail polish, put probe on finger, check pulse (note rate, rhythm and force) to make sure it matches pulse on meter, state results normal range: 97-98% If normal, then state they are not in vasoconstrictive state. If not normal, you would need to check on earlobe. Skin: color, general pigmentation (abcde for moles), temperature, moisture, texture/thickness, edema, mobility/turgor, vascularity/bruising, lesions. Hair: color, texture, distribution and scalp lesions. Nails: shape and contour (profile sign) Clubbing occurs with congenital, chronic and cyanotic heart disease and with emphysema and chronic bronchitis: colour, consistency, capillary refill.
