Explain the relationship between planning and nursing assessment and diagnosis. Assessment of patient will lead to diagnosis, then based on diagnosis, nurse can make a plan to fix the patient"s problem. Determining the urgency and importance of the problem will determine the priority in which the nurse will implement the plan. Determine which problems need to be addressed first depending on its urgency. A goal is a broad statement that describes a desired change in a patient"s condition or behavior. Discuss the difference between a goal and an expected outcome. A goal is what you want to achieve and an expected outcome are things that need to be accomplished to achieve the goal. List the seven guidelines for writing an outcome statement. Outcomes and goals reflect patient behavior and responses elected as a result of nursing interventions. Each goal and outcome should address only one behavior or response. Observable - observe if change takes place in a patients status.
