Mortality and disease rates vary across neighbourhoods in developing countries. Health in certain areas is worse than others. Whether the differences can be explained simply by the association between individual risk factors and health composed with the clustering of individuals with similar risk profiles into the same areas. Or whether they are due to additional effects of a area of residence over and above individual risk factors. Sever designs to study the association between neighbourhood environment and health: Multilevel studies: combine neighbourhood level data with individual health. Ecological studies: relate population characteristics to population health. Individual or in-depth studies: relate perceptions of the local environment to health. Only multilevel studies and individual studies can be used to tease out the contextual and compositional explanations. Challenge is deciding how the neighbourhood should be operationalized. Economic, social-relational, service and physical aspects of the neighbourhood may be important for health.
