Bipolar disorder formerly manic-depressive illness, is a severe biologic illness. It"s chronic, requiring lifelong treatment & rx = mood stabilizers (lithium & many aeds); There is recurrent fluctuations in mood; alternating episodes in abnormally elevated & depressed mood, separated by periods of normal mood. Symptoms usually begin in adolescence or early adulthood; can occur in very young children. Without treatment, mania & depression episodes often last several months & recur more frequently. Types of mood episodes seen in bipolar disorder: pure manic episode (euphoric mania) Symptoms similar to paranoid schizophrenia: hallucinations, delusions, bizarre behavior. Bipolar frequently misdiagnosed as schizophrenia=use antipsychotics=worsen long-term outcome. A distinct period of abnormally & persistently elevated, expansive, or irritable mood, lasting at least 1 week (or any duration if hospitalization is necessary). Increased goal-directed activity (socially /at work or school / sexually) or psychomotor agitation. Inflated self-esteem or grandiosity (engaging in unrestrained buying sprees, sexual indiscretions, or foolish business investments)
