Federal systems: virtues of centralism seem especially pronounced, slower reaction time to medical crisis. The national level government has more economic & administrative capacity to establish complex hc services but the responsibility falls on smaller jurisdictions with less revenue. Rich provinces such as ontario have much of the fiscal ability & technocratic expertise to design & implement hc services while many smaller & poorer provinces struggle to main the status quo. Larger & wealthier provinces are better able to take advantages of the advances in information technology that require heavy capital investment. Severe regional disparities can lead to significant inequities in health & health care between citizens & to the extent that access to health services is viewed as an aspect of citizenship.
