Think about examples from our discussions about governance, mental health policy and drug policy. Canada is a federal country, in which power is split between the national government and the ten provinces. The canada health act is a set of conditions that each province must achieve in terms of health care provision, but ultimately the provinces have autonomy over the delivery of health services. The division of health services between the federal and provincial level has also led to a political power struggle. Although it may be apparent that there is need for policy reform on a national level, the federal government will be hesitant to implement a plan that will reduce the autonomy of the provinces. This has ultimately resulted in discord between the federal and provincial governments which has led to gaps in services, inefficient planning and a loss in quality of care.
