Central focus of nursing can be thought of as the heart of nursing. Profession of nursing is an organism of humans rather than a human organism. The heart of nursing would be the last aspect to be given away. A useful choice meets the ultimate test the life of the profession is assured. Each nurse has her own general notion of what should be central and seen as the heart of nursing. Leadership could be a central feature: inspirational leaders, persuaders, diagnosers, nurse scientists. Transitional discharge based on therapeutic relationships: state of the art. The transitional discharge model (tdm) has been used to facilitate effective discharge from psychiatric hospital to community. The model is based on the provision of therapeutic relationships to provide a safety net throughout the discharge and community reintegration processes. These relationships include both staff and peer involvement.
