Its possible that only a few states will fully construct their own exchanges: states where leaders won"t run exchanges leave residents to sign up on the federal website. Will cost controls work: high stakes for state & national budgets, while the federal government is paying the most to cover lower- & middle-income. Americans, after 2016 state governments could be forced to pay more for expanded. If costs continue to rise, it"ll likely be due to successful efforts by the health care industry to persuade members of congress to give up controls: politicians would have to stand up to lobbyists. Some reasons for cautious optimism: congress has a reasonably good record of standing by its commitments to control medicare budgets, looming government budget deficits give both parties an incentive to support cost controls. Why future elections matter: democrats who are mostly committed to seeing health reform go forward are pitted against.
