For adults, retrospectively looking back so ask fam, friends to report on how was as a child maybe start chore at home but rarely complete it: mental effort. So either inattention alone, hyperactivity alone or both. If look ar same age yrs see diff there maybe observe child in certain settings to confirm what these informants were saying.
