Home and community care: home care includes: nursing, therapies, homemaking, personal support services, community services include non-clinical supports: meals, transportations, supported living, home help, other services: adult day programs, respite. Functions of home and community care: prevent or delay deterioration in functioning, act as a substitute for residential long-term care, substitute for hospital care (after discharge; but also, to prevent admissions and re- admissions) Reliance on informal caregiving is associated with increased risk of institutionalization. Why? if the caregiver does not know how cannot care properly. Factors: paid personal care services to meet the needs of instrumental activities of daily living (iadl, awareness of unmet needs (chen and adams thompson, 2010) Cost analysis: different perspectives, example: using personal support workers to do some of the work of the nurses and have clients pay a user fee. Cost effectiveness: hollander"s research: costs increases over time for those whose housecleaning service was cut compared to those who remained on service.
