GENERAL BACKGROUND:
John is a married 35 year old father of two school aged children.
Wife: Jen (33 years old)
Children: Lindsey (8 year old daughter), Jordan (5 year old son)
He started smoking when he was 16 years old—it as the ool thig to do aog his
high shool fieds at the tie. His fathe also soked at hoe. Johs sokig
gradually increased and for the past 10 years John has smoked one pack of cigarettes
per day.
Johs ife does ot smoke.
Jen worked as an educational assistant at a local elementary school prior to having
children. She is currently a stay-at-home mom. She plans to return to work when
Jordan starts school full time.
When the children are at home John will smoke outside or in the garage, but will smoke
inside the house when his kids are not in the house.
John works full time in the IT department of a local trucking company
It is very stressful in the work environment due to the current economy. John feels a lot
of pressure from his boss to do well and work with the limited resources available at this
time.
Most of Johs o-workers smoke.
Past Medical History
John is aware of some of the negative health implications of smoking, but admits his
knowledge is fairly limited to what he hears his family tell him.
He has a podutie sokes ough in the morning.
Aside from expressing an interest in smoking cessation, John does not have any other
health concerns.
His father smoked for 35 years and recently died from chronic obstructive lung disease
[COPD].
Functional Assessment
Self-Concept:
Has no specific health complaints at the moment.

Is aware that his smoking has the potential to negatively impact his health and that it is
