Defense mechanism against anxiety re conflict (primary gain) Freud - first to talk about etiology, causality of conversion disorder or hysteria because all he saw was young women with the symptoms of hysteria. Punishment of verbal expression of emotion leaves no options, therefore somatic illness gets the attention that they need that was not provided when the verbally said that they need attention and affection. Child learns verbal label to communication emotions, needs. Some do not learn well, continue to use somatic expression of emotion. Initial innate physical expression of emotion triggers response by mom - with appropriate parenting these needs get met. Reinforcement for somatic complaints (attention, nurturance, love) Increased use of somatic complaints to meet needs of affection. Ssd comorbidity: major depression = 87, panic disorder = 45, mania(part of bipolar disorder) = 40, drug and alcohol dependence = 40, phobic disorder = 39, ocd = 27, schizophrenia = 27% Preoccupation with having or acquiring a serious illness.
