Explain the diagnosis (dx) in your own words: Multiple sclerosis (ms) is an autoimmune disease which affects the central nervous system (brain and spinal cord). Ms attacks the myelin surrounding nerves, causing inflammation, scarring and slow and/or absent nerve impulses. Myelin is a sort of protective sheath which covers nerve fibers and enhances nerve conduction. Surgery (sx) or treatment (tx) or diagnostic test. This patient has a tracheostomy related to respiratory dysfunction from ms. This patient also has a g-tube related to gi dysfunction such as severe dysphagia. Explain the sx or tx or test in your own words: Tracheostomy: a surgical procedure to create an opening (aka stoma) through the neck into the trachea which creates a new path for air to reach the lungs. G-tube: a tube inserted through the abdomen which delivers nutrition and fluids directly into the stomach.
