Palpate for moisture, temperature, texture, turgor and edema. Assess trigeminal (cn v), motor strength and sensation, three facial branches: strength: Get patient to close eyes, indicate when touched. Side to side, senses light touch and pain on ophthalmic, maxillary and mandibular areas with wisp of cotton, repeat with splinter blade. Assess trigeminal and facil nerves (cn v and viii: squeeze eyes shut, wrinkle forehead, clench teeth, smile, puff cheeks. Assess corneal reflex: put cotton near side of eye. Assess glossopharyngeal (cn ix) and vagus nerve (cn x: test gag reflex with tongue blade. Assess hypoglossal nerve (cn xii: look for symmetry. Darken room, obtain light, assess optic nerve (cn ll) Shine a light from side of face to the eye, inspect for reaction. Tell patient to look far away (observe for dilation) Then tell patient to look at your finger (observe for constriction) Assess oculomotor (cn iii), trochlear nerve (cn iv), abducens (vi), and extraocular movement: cardinal directions.
