Chronic rejection inflammation, resulting from early tissue injury. Ab mediated, therefore complement activation, and fc receptors on nk cells. Antibodies must be pre existing: natural antibodies and b1 cells, most of these natural antibodies target carbohydrates and sugars, common on bacterial capsules, also common on cells membranes. But we are tolerant to these due to tolerance of self antigens. In some cases, ab to allo-mhc: transplants must be blood group matches. Must be done for blood transfusion and most solid organs, but not the liver. Your blood cells have different sugars which are classified as the ab antigens: these are also found on bacteria, but if you are born with these antigens you can be tolerant to them. Recall rh blood group and pregnancy: rhogam prevents b cell activation and memory cell formation. Human donors are limiting, so we look to other sources of transplants.
