The bacteria that were isolated most often from the infection site and blood cultures include e. coli, streptococcus spp. , and bacteroides spp. However, in patients diagnosed with severe infections, the pattern of bacterial isolates may change and include candida, enterococci, enterobacteriaceae and s. epidermis. Empiric antibiotic therapy for healthcare-associated intra-abdominal infection should be driven by local microbiologic results. To achieve empiric coverage of likely pathogens, multi drug regimens that include agents with expanded spectra of activity against gn aerobic and facultative bacilli may be needed. These include carbapenems (meropenem, imipenim-cilastatin, doripenem), piperacillin- tazobactam, ceftazidime or cefepime in combo with metronidazole.
