Alert = awake & readily aroused, oriented, fully aware of external & internal stimuli. Lethargic or somnolent = not fully alert, drifts off to sleep if not stimulated, can be aroused if name is called but looks drowsy, inattentive, loses train of thought, spontaneous mvmts are decreased. Obtunded (deeper lethargy) = sleeps most of the time, difficult to arouse, needs loud shout or vigorous shake, confused when aroused, speech is monosyllables, mumbled, incoherent. Stupor or semi-comatose = responds only to persistent or vigorous shake or pain, groans, mumbles, moves restlessly, reflex activity is present. Coma = completely unconscious, no response to pain or external stimuli. Acute confusional state (delirium) = clouding of consciousness (dulled congnition, impaired alterness), inattentive, incoherent convo, impaired recent memory & confused w/ recent events, often agitated, visual hallucinations, disoriented. Dysarthria - distorted speech; misuse of words, omitting letters & words. Aphasia loss of the ability to speak or understand speech, as a result of a stroke.
