Level of consciousness pt is awake, alert, & aware from stimuli. Facial expression appropriate to the situation. Mood and affect judge by body language & facial expression. Orientation awareness of the objective world in relation to the self (ie know the date, know who examiner is etc) Power to direct thinking toward an object or topic w/ ability to focus on one specific thing w/o distraction. Immediate memory what is going on; ask pt to recall a statement you just made. Stupor or semi-comatose (asleep & not arousalable but responds to pain) Pd & depression: expression may be flat. Pk & depression: slow & monotone; manic: rapid, loud, pressured. Dysarthria distorted speech; misuse of words, omitting letters & words. Orientation lost in this order: time, place, persons, self. Impaired w/ ppl who experience anxiety, fatigue, drug intoxication, Impairment seen as confusion, negativism, irrelevant replies, digression from initial thought. Affected by head injury, fatigue, anxiety, strong emotions.
