Chapter 39: assessment and management of patients with rheumatic disorders. Increased stress on end of bone &/or cartilage. This leads to disorders of muscles, tendons, ligaments and cartilage. Rheumatic diseases: common clinical presentation is arthritis: inflammation of a joint and pain, more than 100 types of rheumatic disease, marked by periods of remission and exacerbation, problems. Immobility: fatigue, adl"s, altered self-image, pain, sleep disturbance, systemic effects with organ failure. Rheumatic disease process: affects all ages, ethnic groups, women have 2-9 times higher occurrence than men, often follow cycle of remission exacerbation. **autoimmunity leads to destruction of tissue and inflammation. Inflammatory process initiates as normal response to an antigen; process does not stop as it should. Continued inflammation destroys joint cartilage and bone: harmful stimuli formation of t cells (antibodies) immune complexes form. Clinical manifestations: pain, joint swelling, limited movement, stiffness, weakness, fatigue.
