1.)Many studies done WITHIN countries, including high income, middle income and low
income countries (Starf ield et al., 2005; Macinko, Starf ield and Erinoso 2009), show that
areas with better primary care have better health outcomes, including total mor tality rates,
heart disease mortality rates and infant mortality, and earlier detection of cancers such as
colorectal cancer, breast cancer, uterine/cervical cancer and melanoma. (Starfield, 2009, p.
59).
The above statement indicates that the best determinant for heal t h care is pr imary care.
Does this ignore the inf luence of econo mic factors? The author stated that regardless of
income status of the country better health resulted from improvements in primary care, if
this is true why is t here a general consensus about higher economic status better is the health
outcome?
2.) In Starf ield’s article, she mentions thatspecification of the population eligible to receive
services is essential in achieving effective service. Who determines which populations
deserve these services? What measures are available to ensure this is done equitably?
