Senses are used to gather data during the physical exam through practice and repetition applying senses to assess each pt"s health state will improve through the use of inspection, palpation, percussion and auscultation. Inspection: concentrated on watching closely- first the individual as a whole and then each body system. Majority of rn and np don"t examine in clinical setting. Keep in mind that should avoid any distracting noises and ensure that the exam room is quiet, private with good lighting equipment. During exam, shouldn"t have to leave room to find item- all items should be available and easy to reach and laid out in organize manner. Clean stethoscope so it doesn"t become a staph-oscope! . Could be a common vehicle for transmission of infection thus must clean isntrucments with alcohol swab after use in diff pts in an effective control of infection.
