Diabetes mellitus predisposing factor to hf regardless of cad or htn. Etiology is complex & r/t in part to microvascular pathology caused by diabetes: by age 65, 50% of canadians have htn & 90%+ will develop it within their lifespan, other risk factors incl. : cigarette smoking, obesity, advancing age, & high serum cholesterol. Hf may be caused by mechanisms interfering with co regulation. Co depends on: preload, afterload, myocardial contractility, heart rate, metabolic state of individual. Any alterations to these can lead to decr. ventricular function & manifestations of hf. Hf may be divided into two sub groups: Precipitating causes diabetes (high glucose damaging intimal lining of the arteries atherosclerosis) O2 carrying capacity of blood stimulating in co to meet tissue demands. Changes the tissue metabolic rate; hr and workload of the heart.
