Chapter 15Cognitive DisordersDiagnostic issues o for cognitive disorders that have an underlying medical problem as their cause both the cognitive problem and the medical cause are listed on Axis 1 o sometimes the qualifying phrase due to a specified medication condition is usedo in addition the medical condition is causing the cognitive problem is listed again on Axis 3o many of the common neuropsychological disorders are handled in this manner o ex cognitive impairment associated with HIV disease might have the following DSM codeAxis1 Dementia due to HIV disease Axis 3 HIV disease o cognitive impairment associated with Alzheimers diseaseAxis 1 Dementia of the Alzheimers type Axis 3 Alzheimers disease o pathological brain changes that related to the toxic effects of abusing certain substances such as long term excessive alcohol consumptionare handled differentlyo in axis 1 it will include a substance induced persisting amnestic disorder and no diagnosis is giving on axis 3 eg axis 1 alcoholinduced persisting amnestic disorder axis 3 none clinical signs of brain damage o cell bodies and neural pathways destruction is permanent with some exceptions o when brain injury occurs in an older child or adult there is a loss in established functioning o this loss of already acquired skills can be painfully obvious to the victim leading to a psychological burden depression anxiety however it can lead to victim towards rehabilitation o in other cases the impairment may extend to the capacity for realist self appraisal anosognosia leaving these patients relatively unaware of their loses and hence poorly motivated for rehabilitation o damage of brain tissue may involve a wide range of psychological impairments depending on
