Rheumatoid arthritis (ra): an autoimmune, inflammatory disorder; can develop at any age. Has systemic manifestations: fever, weakness, fatigue, weight loss, thinning of skin, scleritis (sclera inflammation of eye), corneal ulcers, vasculitis, & nodules under skin + periosteum. Inflammation begin in synovium synovial membrane thickens & begin to envelop articular cartilage. Important cytokines: 1) tumor necrosis factor, 2) interleukin-1, 3) interleukin-6, 4) interferon gamma: platelet-derived growth factor, & 6) granulocyte-macrophage colony-stimulating factor. 3 classes of drugs: nonsteroidal anti-inflammatory drugs (nsaids, glucocorticoids (adrenal corticosteroids, disease-modifying antirheumatic drugs (dmards) 2 groups , nonbiologic dmards: small molecules produced by conventional chemical techniques, biologic dmards: large molecules produced by recombinant dna technology; Immunosuppressive drugs target specific components of inflammatory process; combined w/ methotrexate. Drug selection: treatment recommend starting early with dmard within 3 months of ra.
