Gender dysphoria: retained in dsm 5, for people suffering identity related distress, emphasis on gender incongruence rather than cross gender identification, can take many different forms and be expressed in many ways, difference depending on age. Decision to retain focus on gender dysphoria but to drop gid: children who experience sense of inappropriateness in culturally prescribed gender role of their sex but who do not experience discomfort with their biological sex should not be considered to have gid, gid in children should be removed, gid label stigmatizes. 6 12 month psychotherapy: focuses on anxiety and depression, also on available options for altering body, usually only hormonal or cosmetic surgery, sex reassignment surgery, next step, existing genitalia is changed. Preoperative factors predicted favourable post surgery adjustment: reasonable emotional stability, successful adaptation in new role for at least one year before the surgery, adequate understanding of actual limitations and consequences of surgery, psychotherapy in the context of an established gender identity program.
