Defect in btk ( bruton tyrosine kinase) Need t cell helper functions for b to go from igm to igg etc, B cell switching is t cell helper dependent. Without t cell helper function, b cell is stuck in igm they cannot switch they produce lots of ab and produce lots of igm -- compensatory increase hyper because too much ab. Cd4 t bind to cd40 ligand to allow for t cell to produce cytokine to allow for b cell to class switch. Defect in t cell dependent b cell activation. Defective switching of b cells to the igg and iga isotypes -- and a compensatory increase in igm in the blood occurs. Common variable immunodeficiency manifests differently in different peoplenand at different stages.
