Cd4 t bind to cd40 ligand to allow for t cell to produce cytokine to allow for b cell to class switch. Defect in t cell dependent b cell activation. Defective switching of b cells to the igg and iga isotypes -- and a compensatory increase in igm in the blood occurs. Common variable immunodeficiency manifests differently in different peoplenand at different stages.
