Doctor assuring the woman that her husband"s doing well and started conversation with her before giving her directions to the hospital room. Boss believed that she gave employee a deadline for the report, but gave unclear instructions of wanting it to be done well, but don"t spend too much time. Employee didn"t clarify the instructions given to him. Boss humiliated him in front of the other co-workers and asked, why does this always happen to me? . Children distracting people behind them as the woman responded, yeah, i love. Woman assumed that the man wanted censorship for their reports, but that wasn"t what the man was suggesting. Pilot ensuring that instructions given by dispatchers is accurate. Transmission receiver never receiving the message to encode.
