Caplan and Hollands 1990 four perspectives
Attempt to use a stronger conceptual framework in the development of what they
refer to as a typology rather than a model !
Framework conceptualized around 2 axis : !
(left, West))One axes refers to theory of knowledge and how knowledge is
generated !
(right, East) Knowledge of health and illness can be perceived as objective and
measurable and is related to the biomedical model at one end of the continuum!
Other end, knowledge is subjective and relates the individual experiences of
health and illness, which may not be quantified !
The other axes refer to how society is viewed and animatedly will impact on health !
((north) Radical change/ social change—states the society is a product of
conflict and those who lack power and consequently have poorer health, struggle
against those who have power!
In order to improve health, the structure of the society must change !
(south) Social Regulation—implies that society has an agreed natural order and
that deviations from social norms go against this agreed order !
Consequently, health related behaviour should be regulated and individuals should
comply with received social norms !
Each quadrant or perspective outlines the essential features about the nature of
health and health promotion practice !
Radical humanist paradigm—suggests that the doctor develops a mutual
partnership with a client and the the clients viewpoint is valued as they experience
health subjectively (between subjective and radical change)!
HP here facilitates personal growth based on the wishes of the client !
Power is vested within the clients but supportive methods are used by
practitioners !
Radical Structuralist Paradigm—proposes that health is measured objectively and
is determined by socio-economic and environmental factors (bw radical change and
objective)!
Health and ill-health are created by the unequal distribution of wealth and power !
HP approach associated with this paradigm is to change the nature of society by
community development or social policy, in order to ensure that power is shared
more equally across society!
Humanist Paradigm—implies that health is defined by the doctor although the
subjective experience of the client is explored !
Aim of this approach is to maximizes a healthy lifestyle—by developing clients
own resources and skills !
Health education is often utilized by practitioners to facilitate this goal !
Traditional Paradigm—related to the biomedical ideas about the nature of health !
