Drugs distribute into compartments in the body where they are either: stored, metabolized, excreted or exert their pharmacological effect. Drug distribution: the movement of drugs into compartments where they are needed. The outer portions have high blood flow but this decreases progressively towards the middle: abscesses (infections with pus) have no blood supply and are therefore difficult to treat with antibiotics. Ability to move out of the capillaries: drug (cid:373)o(cid:448)e(cid:373)e(cid:374)t out of the (cid:272)apillaries i(cid:374)to the i(cid:374)terstitial spa(cid:272)e o(cid:272)(cid:272)urs rapidly due to the (cid:272)apillary (cid:449)all"s permeability; they travel out of the fenestrations. Conditions affecting plasma protein binding: albumin, malnutrition, trauma, aging, liver and kidney disease decrease plasma albumin concentration resulting in an increase in free drug concentration leading to toxicity, ex. The trauma patient has more alpha 1 acidic glycoprotein in their blood and therefore a lower free drug concentration.
